Dr. Yan Kalika of Image Orthodontics is a founder of the Sacramento and San Francisco chapters of Smile 4 A Lifetime. He has dedicated countless hours to promote the program in under-served areas of these communities. And, he personally oversees all the cases for the Smile 4 A Lifetime candidates.

Who We Are
Smile for a Lifetime Foundation (s4l.org) is a charitable non-profit organization that provides orthodontic care to individuals who may not have the opportunity to acquire assistance. Launched in 2008, Smile for a Lifetime Foundation aims to reach individuals with financial challenges, special situations, and orthodontic needs. The Foundation sponsors the orthodontic care of hundreds of patients each year. Smile for a Lifetime Foundation has participating orthodontists throughout the US. Each chapter has its own local Board of Directors who chooses patients to be treated by the Foundation.
It is our mission to create self-confidence, inspire hope, and change the lives of children in our community in a dramatic way. The gift of a smile can do all this for a deserving, underserved individual who, in turn, can use this gift to better themselves and our community.
At Image Orthodontics, we combine a private practice experience with the benefits of multi-doctor collaboration and multiple locations, providing children and adults the highest quality orthodontic care with friendly, highly-trained staff in a comfortable environment. Our orthodontists and staff are here to help our patients realize their potential for a beautiful smile.
Guidelines for Applying for an S4L Scholarship
- Guidelines in applying for braces through Smile for a Lifetime Foundation
- Applicant questionnaire must be handwritten and answered by the applicant
- Applicant must have a reliable source of transportation to and from appointments
- Applicant must have a significant aesthetic need for braces
- Applicant must demonstrate financial need.
- Applicant must be between 11 to 19 years old (For further questions please contact your local Smile for a Lifetime Chapter)
- Applicant must be a currently enrolled student
- Applicant should demonstrate a positive attitude
- Applicant must agree to follow the treatment plan and demonstrate the ability and commitment to make all appointments on time
- Applicant is encouraged to display involvement and leadership in extracurricular activities
Two Letters of Recommendation are mandatory. Please do not submit more than two letters and limit each reference letter to one page each. Please type or print clearly with black ink (no pencil). Letters of recommendations may be written by teachers/coaches, counselors, dentists or spiritual leader etc.
A clear head shot with full smile & teeth showing must be included with application.
The application, letters of reference and pictures will not be returned and will become property of Smile for a Lifetime Foundation.
Applications will be reviewed two times a year. Applications are received on an ongoing basis. Each applicant will be notified of approval or denial after the end of each selection process.

Our Board Members are as diverse as the communities we serve; however, leadership and passion for community improvements are characteristics they all share.
Our experience with Image Orthodontics has been exceptional. My son and I are honored to be part of the Image Orthodontics family. The Dr. gave my son a scholarship to receive braces at no cost which has helped my family financially. We would had not been able to to comfortably afford the excellent care and braces Image Orthodontics provided, if not for the scholarship. Thank you so much for your generosity and care.

About 4 months ago, my 14 year old son starting complaining about pain and discomfort regarding his teeth. I knew there would be no way in the near future that I myself nor his Dad could afford to get his teeth worked on and more importantly being able to afford braces. It was no longer a matter of cosmetic, but about need and my son could wait no longer. I immediately got on the internet and started looking for any ‘free’ dentistry and or orthodontics services in our area. I somehow clicked on the right link and there it was “SMILE FOR A LIFETIME’. I thought to myself this sounds to good to be true, but dialed the number shown to Image Orthodontics. Upon speaking to the receptionist and giving her just a small portion of our dilemma and how my son was starting to feel increasingly more pain, said she would have Nicole call me back and who is the head coordinator for the foundation. Within a few short hours, I received a call from Nicole, who was extremely pleasant and gave me step by step instructions on what to do next and even sent me an email of all the information and how to proceed further. My son and I started immediately on completing our easy application process, gathering a few current pictures, 3 letters of recommendation and a testimonial from my son on how he planned to ‘Pay it Forward’. Lastly, my son was examined by Dr. Milstein, along with a free consultation at Image Orthodontics in Roseville. Being that we applied in October and it was now the second week of November and knowing they only have one more recipient to select for the year, I was starting to have my doubts. It was the week of Thanksgiving and I had just gotten home from work. The phone rang and I was too tired to see who was calling so I just let the answering machine pick up. But as soon as I heard Nicole’s voice on the other side, I immediately grabbed the phone and she asked to talk to my son, which was odd, but then she asked him to put it on speaker for both of us to hear. She did not hesitate and in her sweet voice said “Landon has been selected to receive free braces through ‘SMILE FOR A LIFETIME’!! I of course about fell over and both my son and I hugged, laughed and cried all at the same time. Then we said ‘goodbye’ to Nicole who I believe was still holding on the other side of the phone. We have started the process and my son is getting some much needed dental work done before they can apply braces. We look forward to this ever lasting and life changing experience given to us and most importantly we are ever so grateful to everyone involved with “SMILE FOR A LIFETIME” for making this impossible, ‘POSSIBLE’.
